PHYSICALS/MEDICAL CLEARANCE
Physicals will be done on a yearly basis prior to participation for Non-Traditional Sports and Traditional Sports season. Athletes may not participate without a physical exam (CCCAA Bylaw 3.5.1) and they must be completed by an MD or DO. Athletes are required to have on file a Medical History, the Physical Exam and the Chaffey College Waiver form. Secondary forms for Insurance and Insurance cards, concussion, Athletic Training room Rules, and SportsWare online medical database also need to be completed prior to participation.
Physicals will be offered on a yearly basis through the Athletic Training Department for a $10 fee.
Free On Campus Option:
Student Health Services offers free physicals to students enrolled. This process is a 2-appointment process where they will do height, weight, vision and hearing on the first visit. The 2nd visit will consist of seeing the Medical Doctor where he/she will evaluate all information for the medical physical (bring Athletic’s medical paperwork to SHS as they will not provide it).
To schedule an appointment with Student Health Services have them call 909-652-6331, Mon-Friday. 8am -4pm. Remember Student Health is a free physical.
Off Campus Option:
Athletes may choose to take the physical forms to their own physician. The forms are available below by clicking the Medical Forms tab. The physical must be completed/signed off by a licensed physician (a M.D. or D.O.) and not a Physician Assistant, Chiropractor or Nurse Practitioner.
THE FORM NEEDS TO BE STAMPED/SIGNED BY THE DOCTORS OFFICE (MD or DO) TO BE ACCEPTED.
There is an office in Ontario that performs Sports Physicals and they charge $30.
